By completing and submitting this form, I agree to the following:

1. I give my child permission to attend FSC Youth event at Wild Water Adventure on July 16, 2025.
2. I understand all reasonable safety precautions will be taken by First Street Church and its agents during the events and activities. 
3. I authorize any treatment by an accredited hospital and/or physician deemed necessary for the subject of this release in case of an emergency.
4. I understand the possibility of unforeseen hazards and know the inherent possibility of risk.
5. I agree to waive and release, and to not hold First Street Church, its leaders, employees, and volunteer staff liable for any damages, losses, liabilities, diseases, or injuries incurred by the subject of this form.
